Required Documents for Hamad Health Card

To apply for a Hamad Health Card, you’ll need the following documents:

• Valid Qatar ID (original and copy)
• Passport copy
• Passport-size photo
• Proof of address, such as a utility bill or a tenancy contract (if the utility bill is not in your name). Single workers may be exempt from providing proof of address.

*Make sure to note your home’s National Address, including building, street, and zone numbers, as you’ll need these details during the application process.

Applying for a Hamad Health Card (Offline)

To apply for a health card in person, follow these steps:

• Visit Your Nearest PHCC: You must visit the health centre designated for your area. If unsure, call the PHCC helpline at 107 for assistance.
• Fill Out the Application: Complete a simple form with your details, such as your Qatar ID number, National Address, and contact information.
• Submit Required Documents: Provide the necessary paperwork listed above.
• Pay the Fee: The card costs QR 100 for expatriates, QR 50 for Qataris and GCC nationals, and QR 50 for domestic workers. Payment is usually made via debit/credit card, but it’s wise to carry cash as well.
• Collect Your Health Card: The health centre may issue your card immediately or ask you to return it later to pick it up.

Special Procedure for Single Workers

Single workers must submit their health card applications through their company’s Public Relations Officer (Mandoob) at the Hamad Health Card Printing Office. Workers' Health Centers no longer offer this service.

Applying for a Hamad Health Card Online via Nar’aakom

Thanks to the launch of PHCC’s bilingual Nar’aakom mobile app, you can now apply for a Hamad Health Card online. Here's how:

• Download the Nar’aakom App: Available on the App Store and Google Play Store.
• Select Your Language: Choose either English or Arabic.
• Log in via Tawtheeq: Use your National Authentication System (Tawtheeq) credentials to log in. If you don’t have an account, you can create one through the app.
• Access E-Services: Tap the “E-Services” option at the bottom of the app.
• Apply for Health Card: Select "Apply for Health Card" and complete the required fields.
• Upload Documents: Submit the necessary paperwork digitally.
• Receive Card Information: After processing, you will receive a health card number, which can be used immediately for services.

Hamad Health Card Fees

The fees for obtaining a Hamad Health Card are as follows:

• Expatriates (adults or children): QR 100
• Qatari Citizens (adults or children): QR 50
• GCC Citizens (adults or children): QR 50
• Domestic Workers: QR 50

*Note: Children born in Qatar can receive free medical care during their first year.

Renewing and Replacing Your Hamad Health Card

The card is valid for one year for expatriates and must be renewed annually. Renewal can be done online or at your local health centre, though a new card will not be issued after renewal; your old card remains valid.

If you lose your health card, the replacement fee is QR 100. If the card has expired, the cost for a new one is QR 200.
