Betel leaves, widely known as paan leaves across South Asia, are attracting scientific interest for a very different reason from their traditional culinary and cultural uses: researchers are investigating whether one of their natural compounds could provide clues for developing future treatments for Alzheimer’s disease.

A 2025 study published in the Journal of Aging Research examined hydroxychavicol, a major phenolic compound found in Piper betle. Researchers used network pharmacology, molecular docking and molecular-dynamics simulations to explore how the compound might interact with biological targets associated with Alzheimer’s disease.

The analysis identified 88 possible overlapping targets between hydroxychavicol and Alzheimer’s disease. Among the important proteins highlighted were COMT, GAPDH and HSP90AA1, which have links to processes involving dopamine regulation, amyloid processing, tau metabolism and other mechanisms relevant to the disease.

Researchers also found potential involvement in several signalling pathways, including cAMP, PI3K/Akt, HIF-1, Rap1 and calcium signalling. Computer simulations suggested that hydroxychavicol could bind stably with some of the identified target proteins.

The findings build on earlier animal research. A 2010 study investigated hydroxychavicol in rats with experimentally induced Alzheimer’s-like cognitive dysfunction. The compound was associated with improved cognitive performance and reductions in several inflammatory and oxidative-stress markers in that model.

Together, these findings make hydroxychavicol an interesting candidate for further investigation. However, they do not demonstrate that eating betel leaves can prevent, treat or cure Alzheimer’s disease.

The newer research is largely computational, while previous supportive work includes animal studies. Clinical trials in people would be required to determine whether hydroxychavicol is safe and effective as an Alzheimer’s treatment, what dose might be appropriate, and whether laboratory findings translate into meaningful benefits for patients.

This distinction is particularly important because betel preparations vary considerably. Betel leaf itself should not be confused with betel quid or paan preparations containing areca nut and tobacco, which carry established health risks.

Alzheimer’s disease is a progressive neurological disorder and a major cause of dementia. Current research is exploring numerous biological pathways and therapeutic approaches, and plant-derived molecules remain one area of scientific investigation.

For now, the significance of the betel-leaf research lies not in recommending paan as a remedy, but in identifying a naturally occurring molecule that may help scientists better understand potential therapeutic targets. Hydroxychavicol will require substantially more laboratory and clinical research before its true medical potential is known.

What the research means
